All:

The Season begins in two weeks (weekend of December 5th and 6th) and most
teams still have a lot to do to complete the registration process.

NOTE: This year, we have more teams applying than available spots allow
(Due to Gym space limitations). Priority is based on date payment is
received. Please submit payment ASAP or risk not being able to play, even
if you are registered.

The following is a summary of the items that many teams still need to
complete.

*1. Please make sure your team appears on the Teams Page
- http://www.futsalsj.org/teams.shtml <http://www.futsalsj.org/teams.shtml>*

If not, it means you have not yet paid or your payment has not been
received. If your team is not listed and you believe you have paid, please
contact:

Rosemary Alvarez - disneysoccermom at gmail.com

or, if you need to make a payment, please send a check for $625 payable to
Futsal San Jose to:

Rosemary Alvarez

7234 Via Bella

San Jose, CA 95139

*2. UPDATE YOUR TEAM PAGE NO LATER THAN FRIDAY NOVEMBER 27*

We will begin the scheduling process on Friday November 27th for the 1st 3
weeks of Play (Dec 5/6; Dec 12/13, and Dec 19/20). Accordingly, by 11/27
please make sure you go to your team page on the website and do the
following:

i) Input *BOTH* your Coach and Manager name - It is very important to
list your coach's name, especially for teams where the coach is responsible
for more than one team. If you do not do this, your coach can be scheduled
for more than one game at the same time at different locations

NOTE: The coach's name must be entered exactly the same for each of
his/her teams

Once you complete this look at the "Related Teams" page to be sure each of
your coach's teams are listed under their name
http://www.futsalsj.org/teams_rel.shtml

ii) Bye Requests – at the bye request tab of your team page, enter any
dates/times your team cannot play during the 1st 3 weekends

(1) Please be sure to enter the dates and times correctly and if your team
is not available the entire day, enter 7:00 AM to 10:00 PM to be sure it
covers the entire day.

iii) Friendly v. “unfriendly” opponents - A tab on the team page that
allows you to enter the name(s) of any teams you wish or do not wish to
play.

iv) Please enter your USFF ID # on your team page. You will get this when
you complete the USFF registration process.

Instructions and a link to the USFF page can be found at:
http://www.futsalsj.org/registration_usff.shtml

v) Once you have registered your Coach, Team and Players - Email proof of
age (Birth Certificate, CYSA or US Club player pass, etc) to Rosemary
Alvarez - disneysoccermom at gmail.com

vi) Print Coach and Player passes from team page - It may take a day or
two after sending Rosemary proof of age before you are able to print out
the player passes

NOTE: You cannot upload pictures. Instead, print out the passes, tape a
picture to them, and then laminate each one.

*3. GAME DAY*

Once the schedule is completed and appears on your team page you will be
able to print out a game card for each of your scheduled games and to enter
scores after the game. The home team is responsible to bring a game card
and to have a team representative keep track of the score and fouls. I
encourage home and visiting teams to bring a copy of the game card and for
both to keep track of the score/fouls to help ensure everything is done
correctly.

NOTE: There is a master schedule page on the website that shows all of the
games at each location. PLEASE DO NOT RELY ON THIS PAGE UNTIL AFTER YOU
SEE THE GAMES ON YOUR TEAM PAGE. UP UNTIL THAT TIME, THE SCHEDULE IS STILL
A WORK IN PROGRESS AND SUBJECT TO CHANGE.

Finally, this email may not list everything your team may need to do to
complete the registration process. All of the information and instructions
are on the website. I encourage everyone to read through them. If you
still have questions, please do not hesitate to contact me.

Hope everyone has a wonderful season.

Regards,
Mark Stout
